FDCQA Validation Surveys for Children

School age children are also asked to complete a Validation Survey which has been specifically designed for children. This Survey also contains a series of statements based on the Principles stated in the FDCQA Quality Practices Guide which children will be asked to indicate how they feel about their carer and family day care against a four-point scale: not at all; some of the time; most of the time; and, all of the time. The current statements on this Survey are:

About me and my family

  • My carer listens to me and is interested in what I have to say
  • My carer is nice to my family
  • My carer's family makes me feel welcome
  • My carer and people from the family day care office are nice to each other

About my carer's home

  • My carer's home is comfortable and there is space to do the things I like
  • At my carer's home there are lots of things I like to play with and different things to do

About what I do at my carer's home

  • My carer knows what I like and thinks of interesting things for me to do
  • When someone from the family day care office visits, they take an interest in what I am doing
  • My carer treats me fairly
  • My carer helps me feel good about myself
  • My carer helps me get along with other children
  • At my carer's home I can build and make things
  • At my carer's home I can play outdoors
  • At my carer's home there are books to read and paper, pens and pencils to use when I need them
  • At my carer's home there are activities to try like board games, card games or cooking
  • At my carer's home we can make music, dress-up or do art and craft

About my health and safety

  • My carer makes sure that anything that is broken is fixed quickly or is placed out of bounds
  • My carer makes sure I wear my seatbelt when we travel in the car
  • My carer encourages me to eat healthy food
  • My carer looks after me if I am sick or hurt
  • If I forget, my carer reminds me to wash my hands after using the toilet and before I eat
  • My carer respects my need for privacy when I need to use the toilet or change my clothes
  • There is a comfortable, quiet place in my carer's home where I can rest if I need to
  • I feel safe in my carer's home
  • I can talk to my carer about how I am feeling

About my carer and family day care

  • My carer knows how to care for children
  • Someone from the family day care office often visits or talks with my carer
  • My carer sometimes meets with other people to learn new things about looking after children
  • My carer listens and tries to help me when I have a problem about what happens in care

Validation Surveys must be returned to NCAC in one submission by the due date specified in the letter from NCAC.

Please note: Validation Surveys are designed for data collection to assist NCAC in making an Accreditation Decision. Schemes wishing to survey staff, carers or children during the self-study process are encouraged to design their own surveys which encourage written feedback on various aspects of care and relate specifically to the scheme's practices.

Validation Surveys will be sent to each scheme upon reciept of the Self-Study Report. Sample Validation Surveys are no longer available for download.
